baroreceptors, what nerves, and what do they respond to, and where do they send msg

1. Aortic arch: only responds to high BP, transmits via vagus to medulla
2. Carotid sinus: increases or decreases in BP. Via CNIX to solitary nucleus of medulla

What organs get a lot of blood

Liver(largest share of CO) and kidneys(highest blood flow per gram of tissue)

Peripheral chemoreceptros, where are they and what do they respond to

Carotid and aortic bodies

hypoxia(if PaO2<60), hypercapnia, and acidosis

Central chemoreceptors

Respond to changes in pH and PCO2 of brain interstitial fluid

Cushing rxn, outline what happens and give triad

Increased intracranial pressure constricts arterioles -> cerebral ischemia -> htn(SNS response to the ischemia in brain) -> reflex bradychardia

hypertension, reflex bradycardia, and respiratory depression

respiratory depression, htn, and bradycardia should make you think what?

what kind of htn, abrupt onset w depressed serum K+ levels

hyperaldosternoism

drugs that can cause a Lupus-like syndrome

Sulfasalazine, Hydralazine, Isoniazid, Procainamide

hydralazine increases what

cGMP. Leads to smooth muscle relaxation, mainly of arterioles

toxicity of hydralazine

Lupus-like syndrome and compensatory tachycardia

HTN drugs safe in pregnancy

hydralazaine, methyldopa, labetalol, and nifedipine(and other DHP Ca++ ch blockers)

Two main uses of nitroglycerin

angina and pulm edema(w CHF decompensation especially)

Minoxidil, MOA

opens K+ channels which hyperpolarizes smooth muscle -> relaxation of vascular smooth muscle

used for severe htn

non-DHP Ca++ ch blockers

verapamil and diltiazem(this one also works outside heart some too)

mainly for angina and arrhythmias, but also can be used for htn

DHP CCBs, main uses

nifedipine, etc

htn, agina, and vasospasm(Prinzmetal's angina)

hypertrichosis

hair growth. Side effect of minoxidil

htn drug, ototoxic(esp w aminoglycosides)

Loop diuretics

htn drug, w cyanide toxicity

nitroprusside

htn drug, w dry mouth, sedation and severe rebound HTN

clonidine

htn drug, reflex tachycardia

nitrates, hydralazine, and DHP Ca++ ch blockers

htn drug, to be avoided in pts w sulfa allergy

loop and thiazide diuretics

htn drug, w hypercalcemia and hypokalemia

thiazide diuretic

htn drug, not to give to pt w hx of kidney stones

Loop diuretics (bc Loops lose Ca++)

meaning they lose the Ca++ into the urine.

Tx of malignant htn

think short-acting. Nitroprusside and Labetalol are common choices

also Fenoldopam(D1 agonist)
and Diazoxide(K+ channel closer like minoxadil) are listed in FA

Nitroprusside MOA

increases cGMP via direct release of NO.

short acting. Can cause cyanide toxicity
