b. Diaphragm: dome
shaped device that covers cervix. Use w spermicide FR 16%. Disadv: risk of TSS,
discomfort of insertion
c. Cervical cap:
like tiny diaphragm,adv of extended period of wear(48 hrs). disadv of risk of
TSS, time/skill/effort needed to fit and insert. FR 16% nullipara, 32%
multipara.
d. Sponge: contains
spermicide, fits over cervix. Disadv of risk of TSS, FR 16% nullipara, 32%
multipara.
e. Spermicide and
how often: chemical barrier, reduce motility, using it more than two times a
day can disrupt the mucosa and incr transmission of HIV, avoid if risk of STI
is high, insert high into vagina, some wait 15 min, 29% FR. Nonoxynol 9 most
common.

Oral contraceptives.
Advantages: not directly used w the act of intercourse, convenience of
other benefits like knowing when period will be, less PMS/acne/anemia/ectopic
prg/cancer. Disadvantages: some coagulative AE, daily use. Drugs and classes of
drugs that decrease effectiveness of oral contraceptives: anticonvulsants,
systemic antifungals, antiTB, antiHIV protesase inhib, maybe antibiotics. Important teaching points to increase
effectiveness: take same time each day, use backup plan is taking one of the
above drugs, follow instructions, know what to do if dose is missed. Signs of
potential complications p. 183: ACHES: abd pain/liver/gall problem, chest
pain/SOB/PE, HA/HTN/CVA, eye problems/HTN/CVA, severe leg pain/DVT.

Sterilization: tubal ligation or occlusion makes egg unavailable.
Male vas deferens sealed/tied/cut so sperm is trapped. Permanent. No hormonal
change.
