nursing process: phase assessment

provides direction for a nursing intervention
signs and symptoms
s/s: also called defining characteristics, are the clinical cues that, in a cluster, support the nursing diagnosis
major s/s
s/s that are sually present when the diagnosis exists; must be present to make an actual nursing diagnosis
minor s/s
are evidence for a possible nursing diagnosis; included in ND by the phrase "as evidenced by"
determining interventions
1) desired patient outcomes
2) Etiology of the nursing diagnosis
3) clinical practice guidelines developed from evidence based summaries
