Bromocriptine (Parlodel)
DA agonist, stimulates prolactin inhibition (release of PRIH)
L-DOPA
used for hyperprolactinemia, DA agonist, moderate success
Cosyntropin (Cortosyn)
synthetic peptide of ACTH residues 1-25, stimulates 20 - Alpha- Hydroxylase release
Aminoglutethimide
Inhibits 20 - Alpha - Hydroxylase
Inhibits aromatase that converts androgens to estrogens
Tx= cortisol hypersecretion from adrenal tumors, promising for breast cancer
Metyrapone
inhibits 11 - Beta - hydroxylase, which then prevents formation of cortisol and cortiscosterone. , causing an increase in bloody 11 - Desoxycortisol
Tx= adrenal neoplasms not controlled by ACTH
DOC for Cushing's Disease
Mitotane (Lysodern)
cytotoxic for adrenals
Ketoconazole
antifungal, but inhibits glucocorticoid snthesis at higher doses
Drugs used for Cushing's
Metyrapone
Mitotane
Ketoconazole
Triamcinolone
Synthetic steroid, NO Na+ retention, very antiinflammatory
Prednisolone
synthetic steroid,
Protirelin
TRH analogue, used to Dx primary, secondary or tertiary thyroid deficiency
Thytropan (thyrotropin)
TSH analogue, distinguishes between hypopituitary and primary hypothyroidism
Methimazole (Tapazole)
Propylthiouracil (generic), controls hyperthyroidism until spontaneous remission (used with radioactive Iodine to hasten recovery)
Inhibits thyroid peroxidase
Does NOT affect iodide uptake or hormone release
Propylthiouracil
inhibits peripheral conversion of T4 - T3 by IT - 5 - DI 1
Tx = Grave's disease and Thyroid storm
Sermorelin
GHRH analogue, which causes release of GH AND PRL
Somatostatin
(ghrih0
inhibits insulin/glucagon secretion, which causes increase in FFA and decrease in glucose
inhibits gastrin secretion from pancreas
inhibits secretion of TSH, ACTH
Octreotide
GHRIH analogue, used for
Acromegaly
Diabetes Mellitis
Tumors (Zollinger-Ellison Syndrome, tumor of pancreas)
Somatrem
synthetic GH analogue, from gene splicing
differes from GH by a methionine residue
Somatropin
synthetic GH analogue, from recombinant DNA
ientical to natural GH
20 - Alpha Hydroxylase
Enzyme for conversion of Cholesterol to Pregnenolone (Pregnenolone then converts to Cortisol, Corticosterone or Dehydroepiandosterone)
RATE LIMITING STEP in synthesis of Steroids
11 Beta Hydroxylase
Converts 11 - Desoxycortisol into Cortisol, inhisibted by Metyrapone
Forteo
Recombinant human PTH,
chronic high doses of PTH lead to activation of osteoclasts, but
intermittantly (every other day) administered PTH activates osteoBLASTS
Calcitonin- Therapeutic uses
a) Effective in lowering hypercalcemia in
hyperparathyroidism, idiopathic hypercalcemia, and
vitamin D intoxication. b) Postmenopausal osteoporosis (not really used for anymore) c) Paget's Disease (DOC)
hyperparathyroidism, idiopathic hypercalcemia, and
vitamin D intoxication. b) Postmenopausal osteoporosis (not really used for anymore) c) Paget's Disease (DOC)
Vitamin D
Vitamin D is required for calcium absortpion and bone mineralization. Recommended to patints with inadequate sn exposure or dietary intake.
Pathway= Skin-Liver-Kidney, which makes 1,25 Dihydroxycholecalciferol
Raloxifene (Evista)
Selective Estrogen Receptor Modulator (SERM), for tx and prevention of osteoporosis. Estrogenic effects on bone, antiestrogenic effects on breast and endometrium.
Bisphosphonates
Alendronate (Fosamax_ and Risedronate (Actonel)
tx of osteoporosis and Paget's Disease (DOC)
MOA: bind to hydroxyapatite at sites of active bone resorption, inhibiting osteoclasts (cause their death). incorporated into bone
Major side effect for bisphosphonates
osteonecrosis, due to long duration of action with poor circulation to site. Oral bisphosphonates stay around 10%, however.
Alendronate
FOSAMAX, Bisphosphonate, used to tx Paget's disease (DOC) and osteoporosis
40 mg daily for 6 months
Bisphosphonate drugs
Alendronate (FOSAMAX)
Risedronate
Pamidronate
Zoledronic acid
CI for Forteo
Paget's disease of bone, open epiphyses,
Tx of Choice for Paget's Disease
Bisphosphonate therapy, QID for 6 months
Osteoporosis generally weekly, and treated for 2-3years
